Across the life science industry, leadership teams can point to an impressive list of digital and AI milestones: enterprise licenses in place, global platforms live, pilot results presented at steering committees, and technology roadmaps aligned to corporate strategy. Yet when critical decisions are examined in clinical development, safety, regulatory, and medical affairs, much of the day‑to‑day work still relies on legacy processes, local spreadsheets, and informal workarounds.[1][2][3]

Access to technology has improved dramatically; adoption in core workflows has not kept pace.

The access–adoption gap and why it matters now

For large and mid‑cap pharmaceutical, biotech, and medical device companies, an implicit assumption has often been that once teams secure access to the right platforms and tools, value creation will follow. Over the past five years, that assumption has quietly eroded. Hospitals have expanded predictive AI in areas such as sepsis surveillance, deterioration detection, and operational forecasting, yet adoption and impact have been uneven, with governance and oversight now receiving increasing attention. The illusion of progress: When “live” does not mean “used”

Within complex life science organizations, technology progress is often tracked using metrics that are straightforward to report:

  • Number of platforms live across regions or business units.

  • Percentage of users provisioned with access or training completed.

  • Volume of data ingested into new repositories.

  • Count of AI pilots launched or completed.

These metrics are useful, but they are fundamentally measures of access and activity, not adoption and impact. A platform can be globally deployed, fully licensed, and technically integrated while exerting minimal influence on how decisions are made in everyday work.[1][3]

The trajectory of sepsis prediction tools in US hospitals illustrates this gap. The Epic Sepsis Model, implemented widely as part of a commercial electronic health record, was designed to identify inpatients at risk of sepsis. An external validation study at a large academic medical center found that the model had limited sensitivity for detecting sepsis and generated alerts for many patients who did not ultimately develop the condition, raising concerns about its clinical utility and the potential for alert fatigue. Commentaries summarizing this work have highlighted that such models, when deployed at scale without sufficient local evaluation and governance, may deliver fewer benefits than expected at the bedside.[7][8]

A 2025 analysis of hospital AI programs described a similar pattern across other applications. Health systems rolled back or retired some documentation assistants, billing automation models, and operational optimization tools after initial deployments, often because implementations proved more complex than anticipated, end‑user adoption lagged, or outcomes did not match early projections. Inside life science companies, the same dynamics are less visible externally but no less real. Clinical development groups may have broad access to analytics platforms, yet protocol design, site selection, and enrollment decisions continue to be driven primarily by historical templates and expert opinion. Regulatory teams may have structured content tools available but still rely on manual document assembly and email‑based reviews. Medical affairs and commercial teams often maintain omnichannel systems and real‑world data dashboards while defaulting to local spreadsheets, slide decks, and traditional engagement cycles.[2][3][1]

From a distance, dashboards and program updates show steady progress. At the point of execution, day‑to‑day behaviors that determine competitive advantage have changed far less than expected.

Why access is easier than adoption in life sciences

In principle, access and adoption should move together. In practice, several structural and behavioral factors make access far easier to achieve than adoption, especially in the life science industry.

Structural realities inside large and mid‑cap organizations

For large global enterprises and mid‑sized multinationals, acquiring and provisioning technology is primarily an engineering and procurement challenge. Budgets can be allocated, vendors contracted, integrations built, and user licenses distributed across business units. Global functions and centers of excellence are designed to deliver capabilities at scale and report progress consistently.[2]

Changing how thousands of people across regions, functions, and partners actually work is far more complex. Clinical development, pharmacovigilance, regulatory, medical affairs, and commercial teams each operate within their own systems, incentives, and governance structures. Multiple overlapping programs can introduce tools that address similar pain points from different angles, producing a landscape where teams have access to several options but lack a clear, shared path for which one should anchor the primary workflow.[3][1][2]

External partnerships add another layer of complexity. Contract research organizations, technology providers, data vendors, and alliance partners often bring their own platforms and processes. In this environment, responsibility for driving adoption becomes diffuse: central teams own contracts, implementation partners manage deployment, and local functions are expected to “use” the tools while also meeting demanding operational and regulatory targets.[1][2]

Risk culture and behavioral dynamics in life sciences

The life science industry operates under intense regulatory scrutiny and considerable stakes for patients, providers, and shareholders. Predictably, this has created strong risk management cultures that prioritize reliability, compliance, and control. These cultures are essential, but they also interact with well‑documented behavioral patterns in ways that make adoption particularly difficult:[1]

  • Loss aversion: When teams evaluate new tools, potential downside risks errors, regulatory concerns, timeline disruptions often loom larger than upside gains, especially in clinical development and safety.[1]

In healthcare delivery, these dynamics are reflected in how hospitals approach predictive AI. The American Hospital Association has noted that while predictive models are gaining traction, many organizations still lack mature oversight structures to evaluate model performance, manage risk, and integrate tools into clinical and operational workflows. Recent reviews of clinical AI reach similar conclusions, pointing out that broader adoption is often constrained not by algorithmic innovation but by governance, workflow fit, and alignment with frontline needs.[6][10][4][5]

The consequences of confusing access with adoption are not evenly distributed. For heads of regulatory, pharmacovigilance, and quality, these patterns limit the ability to scale submissions efficiently, respond quickly to new requirements, and demonstrate the full value of digital transformation efforts. The organization bears the cost and complexity of new capabilities without fully escaping the constraints of legacy processes.[2][3][1]

For medical affairs, commercial, and market access leaders

On the customer‑facing side, medical affairs and commercial teams increasingly rely on digital engagement, real‑world data, and AI‑enabled insights. Investments include omnichannel platforms, advanced segmentation, dynamic content management, and analytics for HCP and patient engagement.[3][1]

Here, too, access frequently outpaces adoption:

  • “Single source of truth” systems exist for scientific content and field insights, but teams do not consistently trust or use them as the default reference, maintaining local slide decks and offline notes instead.[1]

  • Omnichannel capabilities are underutilized; certain markets or brands rely predominantly on traditional channels despite having access to more sophisticated tools and data.[3]

  • Real‑world data platforms are established, but the outputs are only partially integrated into value propositions, pricing and access narratives, or ongoing evidence strategies.[2][1]

The impact is felt most acutely in crowded therapeutic areas where differentiation depends not only on product profiles but also on the quality and coherence of engagement and evidence. Increased spending on digital and data does not automatically translate into a step change in market performance when frontline behavior changes slowly.[2][1]

Strategic and financial consequences for senior leadership

For the C‑suite and senior functional leaders, the access–adoption gap is a strategic and financial concern, not a narrow implementation topic.[2][1]

Distorted ROI and technology decisionmaking

Repeated cycles of technology deployment without deep adoption also have cultural consequences. In competitive talent markets, particularly in hubs such as Boston and other global centers, ambitious professionals increasingly seek environments where technology is meaningfully embedded in daily work. Organizations that consistently conflate access with adoption may find it harder to attract and retain leaders who expect digital capabilities to be fully leveraged, not merely procured.[2][1]

Why the access–adoption gap belongs on the leadership agenda now

Healthcare systems are already demonstrating what happens when access to AI outpaces thoughtful adoption: implementation experiences have prompted hospitals to refine governance structures, revisit workflow design, and in some cases modify or scale back deployments. Large life science organizations face analogous risks as portfolios of AI and digital capabilities grow more complex.[9][10][4][5][6][2][1]

The central question is no longer whether an organization or patient population has access to the right tools. It is whether those tools have genuinely changed how critical decisions are made in clinical development, regulatory, safety, medical affairs, and commercial functions.[3][1]

Recognizing where access is being mistaken for adoption is integral to obtaining a realistic view of digital maturity, strategic risk, and competitive positioning. Organizations that can see this gap clearly are better placed to decide where to double down, where to reset expectations, and where to rethink current approaches before committing further capital and leadership attention.[2][1]
